Email Address: The email address you will use to log in and receive verification codes.

Username: Required. 150 characters or fewer. Letters, digits and @/./+/-/_ only.

Password:

• Your password can’t be too similar to your other personal information.
• Your password must contain at least 8 characters.
• Your password can’t be a commonly used password.
• Your password can’t be entirely numeric.

Password confirmation: Enter the same password as before, for verification.

Profile Picture: Optional profile photo. JPG or PNG, up to 1 MB — it will be resized automatically.

Your Name: Your full first and last name.

Address: Your home street address.

City: The city you live in.

Zip Code: Your 5-digit home zip code.

Phone Number: A phone number where you can be reached.

Child — First Name: Your child's first name.

Child — Last Name: Your child's last name.

Second Child — First Name (optional): First name of a second child in the program, if any.

Second Child — Last Name (optional): Last name of a second child in the program, if any.

Third Child — First Name (optional): First name of a third child in the program, if any.
